CUSTOMER STATEMENT- CUSTOMER STATEMENT PRIOR CLAIM CL10608679 - ACCEL ISSUE VEHICLE FEELS LIKE IT WANTS TO STALL OUT WHEN ACCEL CHECK ENGINE LIGHT ON NOTICED BEGINNING OF JAN HASN'T DRIVEN VEHICLE IN ABOUT 3 MONTHS. SHOP FINDINGS- ENGINE TORN DOWN TO FAILURE, SHOP IS STATING THE CONNECTING ROD FAILED ON CYLINDER 3 CAUSING DAMAGE TO CYLINDER 6. SHOP IS STATING NO SIGNS OF COOLANT OR WATER INTRUSION/BREACHING OF CYLINDER HEAD GASKET. SHOP STATES NEEDS ENGINE REPLACMENT ** HAVE INSPECTOR CALL TO SET UP APPOINTMENT WITH THE SHOP ** HAVE TECHNICIAN DEMONSTRATE FAILURE / VERIFY/ DETERMINE CAUSE OF FAILURE. *** NOTE- THE ORIGINAL CLAIM FROM PRIOR SHOP THAT IS CLOSED-SAID COOLANT WAS EMPTY AND THE OIL LEVEL WAS 2 QUARTS OVER FULL. CURRENT SHOP STATES NO BREACH OF HEAD GASKET OR ISSUES WITH HEAD. THEY ARE SAYING THE CONNECTING ROD FAILED. SHOP MUST SHOW HOW THE CONNECTING ROD FAILED CAUSING IT TO BEND AND CONTACT CYLINDER 6 ROD AND CYLINDER WALL, WITHOUT COOLANT/WATER INTRUSION. PLEASE INSPECT ROD AND BEARINGS FOR DAMAGE FROM LACK OF LUBRICATION OR COOLANT INTERMIX. PLEASE INPSECT ALL PISTON CROWNS FOR LACK OF CARBON ON AFFECTED CYLINDERS . INSPECT ALL PISTONS FOR COMPARISON AND PROVIDE PICTURES FOR EACH PISTON AND THE CYLINDER HEADS SHOWING VALVE FACES FOR POSSIBLE STEAM CLEANING FROM COOLANT/WATER, OR CONTACT WITH PISTON. VERIFY OIL PICKUP IS CLEAR AND PROVIDE PICTURE PLEASE. VERIFY HEAD GASKET DID NOT BREACH/LEAK. PROVIDE PICTURE OF CYLINDER HEAD GASKET, BOTH SIDES OF FAILED CYLINDER BANK. IF CODES LISTED IN CAR FOR FAILURE, SHOP NEEDS TO DEMONSTRATE ALL TESTING, HOW THE VERIFIED FAILURE AND LIST STEPS , VERIFY CURRENT, ACTIVE AND IF ANY TESTING PER CODE TO SHOW FAILURE. SUPPORT FINDINGS WITH PHOTOS: PLEASE LABEL ALL PHOTOS CHECK FOR RUST AND CORROSION, LACK OF LUBRICATION, OVERHEATING, COMMERCIAL, UBER, LYFT, TAXI USE, CONTINUED OPERATION OR MODIFICATIONS. DID THESE MODIFICATIONS AFFECT THE FAILURE?

Failed Components and Extent of Damage
THE #5 CONNECTING ROD IS BROKEN OUT OF THE PISTON AND IS JAMMED IN THE #6 CYLINDER BETWEEN THE CRANKSHAFT AND #6 CONNECTING ROD. THE #6 PISTON IS IN PIECES IN THE OIL PAN AND THE ROD IS BENT BUT STILL CONNECTED TO CRANKSHAFT. THE #6 ROD BEARING DOES NOT APPEAR SPUN. THERE IS IMPACT TO THE BLOCK FROM THE BROKEN ROD. THE #5 ROD CAP FOR THE CONNECTING ROD IS IN THE OIL PAN AND IS IN WHOLE CONDITION. THERE IS ALSO A BROKEN ROD BOLT AND THE BEARING IS IN THE PAN FOLDED OVER AND DISTORTED. THERE IS NO SCORING ON THE INSIDE OF THE ROD CAP FROM THE BEARING BEING SPUN. NO SIGNS OF IMPACT TO THE CYLINDER HEAD COMBUSTION CHAMBERS OF THE DAMAGED CYLINDERS. THE VALVES AND VALVE SEATS ARE INTACT. NO SIGNS OF FAILED VALVE SPRINGS OR VALVE TRAIN COMPONENTS. THE ENGINE IS CLEAN, NO SLUDGE OR VARNISH. NO EVIDENCE OF LACK OF LUBE OR OVERHEAT. NO EVIDENCE OF LACK OF MAINTENANCE.

Cause of failure
ENGINE HAS EXTENSIVE DAMAGE MAKING THE ROOT CAUSE DIFFICULT TO DETERMINE. IT APPEARS THERE WAS A FAILURE OF THE #5 CONNECTING ROD BOLT WITH CONTINUED OPERATION AT A HIGH RATE OF SPEED UNTIL THE ROD HAMMERED OUT OF THE PISTON, BROKE OFF THE CRANKSHAFT AND JAMMED INTO THE #6 CYLINDER BREAKING THE PISTON AND IMPACTING THE BLOCK.
Recommendations for Repair
REPLACE ENGINE.
